UK – cruise ship grounding
The UK Marine Accident Investigation Branch (MAIB) issued the report on its investigation of the grounding of the cruise ship Hamburg on charted rocks near the New Rocks buoy in the Sound of Mull, Scotland on 11 May 2015. There were no injuries, but the vessel incurred significant racking damage. The primary cause of the casualty was the failure to apply bridge team management tools effectively, either before or after the grounding.
