Australia – ship banned for three months

clip_image011 The Australian Maritime Safety Authority (AMSA) issued a news release stating that the cargo ship Thorco Luna has been banned from Australian ports for a period of three months after it was detained for the third time this year for consistently failing to meet international standards. Australia – collision due to lookout failures

clip_image011[1] The Australian Transport Safety Bureau (ATSB) issued the report of its investigation of the 23 June 2015 collision of the bulk carrier Jag Arnav and fast utility vessel Total Response about 26 NM north-west of Bunbury, Western Australia.  Jag Arnav sustained minor damage and no injuries to the crew.  Total Response sustained structural damage to its bow and deck, and two crew members received lacerations and abrasions. Both vessels were able to resume their passage. The probable cause of the collision was that a proper and effective lookout by all available means as required by the regulations was not being maintained on either vessel.
